Person-centred care planning: a practical guide for care teams

What 'person-centred' really means in a care plan, the difference between a living plan and a dusty one, and how digital tools keep plans current and inspection-ready.

Every care service writes care plans. Far fewer keep them genuinely person-centred and genuinely current. The gap between those two things is where care quality, and inspection outcomes, are won or lost.

What "person-centred" actually means

A person-centred plan starts from the person, not the paperwork: what matters to them, their history, preferences and goals, and only then how the service supports those. The test is simple, if you read the plan, do you meet a person, or a list of tasks?

The living plan vs the dusty plan

A dusty plan is written at admission, filed, and rediscovered at review time, or worse, at inspection. A living plan changes as the person changes: it's reviewed on schedule, updated when needs shift, and actually read by the people delivering care each day.

  • Scheduled reviews that prompt you, so nothing drifts silently out of date.
  • Linked to assessments, so the plan reflects evidenced risk (falls, nutrition, pressure areas), not guesswork.
  • On the floor, open on a phone or tablet where care is delivered, not locked in the office.

Writing a plan staff will use

  • Use plain language. If a new carer can't act on it, it isn't working.
  • Be specific: "supported to walk to the dining room using their frame" beats "promote mobility".
  • Involve the person and their family, their words carry weight with inspectors and with the team.
  • Connect the plan to daily records, so what's planned and what actually happened sit side by side.

When plans are digital, reviews are prompted automatically, every version is kept, and care staff always open the current plan, not last quarter's. Assessed risks flow straight into the plan, and the whole team, plus permitted family, works from the same up-to-date picture. The result is care that's more personal and evidence that's always ready.
